Job Summary and Responsibilities

The RN Care Coordinator is responsible for performing utilization review activities, overseeing the progression of are and transition of care planning for identified patients requiring these services. The RN Care Coordinator performs this role to meet the individual's health needs while promoting quality of care, cost effective outcomes and by following hospital policies, standards of practice and Federal and State regulations. The position's emphasis will be on care coordination, communication and collaboration with nursing, physicians, ancillary departments, insurers and post acute service providers to progress the care toward optimal outcomes at the appropriate level of care. The RN Care Coordinator advocates for the patient and family by identifying, valuing, and addressing patient choice, spiritual needs, cultural, language and socioeconomic barriers to care transitions. In addition, the RN Care Coordinator strives to enhance the patient experience.

 

We are offering a $25,000 Sign On Bonus to hires who meet the eligibility requirements. We also offer additional pay for participation in our clinical ladder as well as referral bonuses, excellent benefits, and relocation assistance. 

Job Requirements

  • Unencumbered license to practice in the State of Arkansas 
  • 3 years of acute care nursing experience required
  • BSN preferred, or Associate Degree with CCM or ACM Certification within 2 years of hire date
  • Previous case management, utilization management, or discharge planning experience strongly preferred
