Job Summary and Responsibilities

Provides direct social work services including assessment, treatment planning, intervention, counseling, and recommendations to individual patients (adolescents, adults, elder) and their families/significant others and/or groups of patients. is knowledgeable in areas such as active listening, social perceptiveness, critical thinking, and coordination. The social worker has the ability to carry the role of advisor, therapist, administrator and clinician.

 

Essential Functions

  • Conduct and document psychosocial assessment of patients
  • Share responsibility in identifying appropriate decision makers if patient is without capacity, educating all in patients’ rights and encouraging participation in decision‐making and care
  • Provide liaison services and referrals to hospital resources and community agencies, including medication assistance programs
  • Identifies mental health needs of patients and Provides counseling and referral as indicated
  • Collaborates with interdisciplinary healthcare team members to ensure continuity and coordination of patient care throughout the continuum and into the community by coordinating placement with home health agencies, long term acute hospitals (LTACs), skilled nursing facilities (SNF) and assisted living facilities, nursing home placement, acute rehab, psych placement, and custodial care facilitation
  • Serves as a resource to the patient, family, and healthcare tem regarding emotional and behavioral issues and refers to community services available to adolescent, adults and elders
  • Remains current of Medicare and Medicaid laws and guidelines, Medicare Conditions of Participation, managed care contracts, and other legal and regulatory requirements as they relate to health care social work services. Assures patients are informed of rights and responsibilities as participants in these programs

Job Requirements

Education and Experience Required:

  • Masters Degree from a Council on Social Work Education accredited graduate school of social work
  • Three (3) years clinical social work - OR -
  • Three (3) years allied health practice, with hospital or medical background or experience

Licensure and Certifications Required:

  • Master Social Worker: TX (MSW:TX)

Knowledge, Skills, Abilities, and Training:

  • Creative problem resolution skills, excellent interpersonal and team work skills, a positive professional and personal image, and a highly independent and organized individual, with excellent verbal and written communication skills.
  • Computer skills; must be able to document in electronic medical records.
  • Exercises knowledgeable, independent judgment on routine and non‐routine matters.
