This position is also a dual role! Not only will you be helping our value medicare patients you will also be helping patients transition after getting out of the hospital! As the person who is a Care Navigator, you are the vital link in the patient's journey, the person who connects the dots and ensures a smooth "handoff" as they recover. You'll work with a diverse team of physicians, nurses, and social workers to coordinate care, educate patients and families, and ultimately, prevent unnecessary hospital readmissions. This role is perfect for a highly organized and empathetic professional who thrives on variety and is ready to champion patient wellness.
