Supports care management and coordination activities in an outpatient setting. Partners with healthcare team to maximize patient engagement and patient outcomes. Identifies patients overdue for visits, labs,
or referrals and initiates patient supportive services to reduce negative impact on patients' health and promote efficacy of clinic resources.

 

  • Acts a liaison with clinic providers, specialty providers, office staff, hospitals, pharmacies, and patients to coordinate care.
    • Reviews medical records for preventive and chronic health care needs and appointments.
    • Responsible for collecting records and updating the EMR, communicating with patients via phone, portal, and mail, placing orders, scheduling of tests and/patient appointments.
    • Tracks orders and referrals through to completion and follows up with patients or clinics as appropriate.
    • Document ongoing status, interventions, patient response and outcomes in accurate, timely manner.
    • Utilizes multiple EMRs and Payer portals to collect and submit records to improve contract performance.
    • Works with IT resources to accurately maintain records when appropriate.
    • Ability to self-motivate, prioritize, and be willing to invest in change processes to improve efficiencies.
    • Seeks learning experiences that reflect current practice to maintain skills and competence in practice or role performance.

 

 

Job Requirements

In addition to bringing humankindness to the workplace each day, qualified candidates will need the following:

  • Required High School Diploma/G.E.D.
  • Minimum 3 years of experience in an outpatient medical practice
  • Proficient computer skills – data entry, retrieval, and report generation, excel and virtual meetings
  •  Experience with Epic preferred
