Job Summary and Responsibilities

Functions independently and consistently in the delivery of dialysis therapy to all types of patients, including those in complex situations with overt and covert needs and / or complications of treatment through
assessment, planning, implementation and evaluation. The registered nurse will be responsible to prescribe, delegate and coordinate nursing care. This position requires providing care/service from a chronically ill to a critically ill through rehabilitating adolescent through geriatric patient population in a manner that demonstrates the functional/ developmental age of the individual served. The registered nurse will incorporate multiple disciplinary care and management with involved health care team members.

  • Adheres to established Hospital/departmental policies and procedures, objectives, Quality Assessment and Improvement Program, legal, safety, environmental, infection control standards, Nurse Practices Act and Patient's Bill of Rights.
  • Applies clinical knowledge and expertise for coordinating patient care by regularly planning and evaluating the care of patients.
  • Establishes and maintains professional and collaborative relationships with other staff members, physicians, patients, patient family members, and the public as appropriate, while assuring confidentiality of all patient information.
  • Demonstrates effective nursing judgement and a working knowledge of the nursing process.
  • Provides guidance to interdisciplinary team members by supervising nursing interventions and assisting with related tasks.
  • Provides leadership by establishing priorities and coordinating nursing care.
  • Documents and communicates all observed symptoms, reactions, treatments and changes in the patient condition to the appropriate member of the interdisciplinary health care team.
  • Provides for teaching based on identified need.
  • Coordinates plan of care with other members of the interdisciplinary health care team to achieve desired goals and outcomes.
  • Communicates the plan of care to the patient and care partner(s) for the active involvement of the patient in the evolution of the plan throughout hospitalization.
  • Conforms to legal requirements of nursing practice and Hospital policies and procedures.
  • Administers medications to assigned patients as directed and assesses and documents patient responses; initiates appropriate interventions per Hospital/Unit policy.

Job Requirements

Required:

  • Registered Nurse (RN) - Associates Degree
  • 2 years of recent (within 6-10 months) RN dialysis experience
  • Minimum of 2 years acute care experience
  • Must demonstrate high proficiency level in the area of hemodialysis, critical care nursing assessment and intervention.
  • Electronic Medical Record (EMR) experience;
  • Must have a thorough knowledge of nursing principles and procedures, a thorough knowledge and skills and the techniques of the nursing care of the dialysis patient as well as critical care patients
    including expertise in the use of dialysis equipment, resuscitation equipment and monitoring equipment such as EKG.
  • Basic Life Support - CPR (BLS-CPR)
  • Advanced Cardiac Life Support (ACLS)
