Job Summary and Responsibilities

As our Community Health Worker, you will be a vital link between individuals and essential community resources, actively working to reduce health disparities and improve access to support services.

Every day, you will proactively engage with clients, assessing their needs and identifying pathways to care and support. You'll connect individuals to relevant health, social, educational, and employment resources, providing guidance and practical assistance to overcome logistical barriers.

To be successful in this role, you will possess a profound understanding of diverse community needs, exceptional problem-solving abilities, empathy, and resilience. Your dedication to empowering clients and removing barriers to care will be paramount.

  • Advise clients or community groups on issues related to diabetes and obesity, improving general health, diet and exercise, prevention, sanitation or hygiene, self care, social issues, education, childcare or problem solving.
  • Act as a resource to all clients, providers and the community. Attend community meetings and health fairs to understand community issues and build relationships with community members. Distribute flyers, brochures, or other informational or educational documents to inform members of a targeted community. Refer community members to needed health services.
  • Advise clients or community groups to ensure parental understanding of the importance of childhood immunizations and how to access immunization services.
  • Advocate for individual or community health needs with government agencies or health service providers.
  • Identify or contact members of high-risk or otherwise targeted groups, such as members of minority populations, low-income populations, or pregnant women. Identify the particular health care needs of individuals in a community or target area. Interpret, translate, or provide cultural mediation related to health services or information for community members.
  • Provides one to one coaching to those individuals desiring this service.

Job Requirements

Required

  • High School Graduate and Experience working in the community., upon hire or
  • High School GED, upon hire

Preferred

  • Bachelors Other and teaching, upon hire
