The acute Rehab Admissions & Discharge Social Worker is responsible for performing social work assessments, interventions as needed, and planning for patients discharge needs. They are dedicated to patient and family centered care that values personal self-determination, skilled at managing multiple priorities at once, and engaging in creative, compassionate, and ethical problem solving.
Has excellent understanding of rehab admission criteria, both pre-admission, admission and discharge and insurance payor. Requirements for rehab admission. Measurement: Chart Review. Obtains prompt authorization from payors prior to patient admission to the Rehabilitation Center. Collaborates effectively with payors and provides timely documentation for admission authorization review. Measurement: Chart Review.
