Job Summary and Responsibilities

Educator develops, coordinates and implements education programs and support services to serve the needs of community members. Provides knowledge that is relevant to coping with illness, promoting recovery to healthful living and appropriately involves the consumer in his/her healthcare decisions. Ensures customer's initial contact with the Resource Center is one that fosters a sense of security and confidence.

  • Assess the needs of the community through involving target population and partner organizations in program
    planning. Formulate appropriate and measurable program objectives.
  • Acts as a facilitator and consultant to enhance education goals in caring for hospital customers (in-patient,outpatient and community). Use a variety of methods to implement strategies, interventions and programs.
  • Plans curriculum content and sequence and other activities for customers and their families.
  • Ensures patient/customer's initial contact with the Resource Center is one that fosters a sense of security and confidence.
  • Provides one to one coaching to those individuals desiring this service.
  • Develops, coordinates and implements education resources, programs and support services to serve the needs of our community as identified by the Community Health Needs Assessment.
  • Consults with internal customers concerning instructional needs of patients and the community.
  • Complies with physician requests for patient instruction and coordinates instructional activities of multidisciplinary team members.
  • Maintains current, accurate records of attendance/participation and outcomes in educational programs.
  • Review research and evaluation procedures to design data collection instruments.
  • Collect data on every program and report outcomes on a regular basis. Interpret results from evaluation.
  • Utilize results to enhance future programming.
  • Respond to requests for health information from the community and colleagues.
  • Establish consultative relationships with health partners.

Job Requirements

Minimum:

  • Bachelors in Health Education or related field.
  • Experience in teaching and working in the community.
