Job Summary and Responsibilities

As a Caregiver, you will be a vital member of our healthcare team, dedicated to optimizing patient safety and well-being. You will be part of our highly specialized team of patient caregivers, working under the direct supervision of a Registered Nurse.

Every day, you will maintain continuous oversight of assigned patients, directly assisting with their personal hygiene, mobility, and requests, while diligently monitoring their physical and emotional condition.

To be successful in this role, you must be compassionate and comfortable interacting with patients who might be emotional, panicked, stressed, or in pain. While performing duties under various circumstances, you will be eager to work calmly and efficiently to respond to situations, ensuring the best possible outcome for the patient.

  • Monitors and records resident vital signs, weight, positioning, ADL's, activity and blood glucose readings per Nurse/Supervisor instructions, and standards of care; notifies Shift Lead/ RN of abnormalities; records readings in an accurate and timely manner.
  • Maintains accurate documentation including documenting clinically relevant findings in the resident's record according to protocol.
  • Initiates activities as scheduled, and maintain an activity-based structure during down time.
  • Effectively assists with resident admission, discharge and transfer, including settling residents appropriately, orienting residents to rooms/routines, obtaining vital signs/specimens/monthly weights as directed, and assisting staff/family with discharge preparations. Competent in use of behavioral interventions.
  • Demonstrates efficiency and accuracy on all tasks; completes all tasks in a timely manner; promptly responds to requests; devotes appropriate attention to all responsibilities.
  • Communicates appropriate information to Shift lead, Supervisors, hospice agencies, and family members.

Job Requirements

Required

  • Six (6) months of caregiving experience and
  • Basic Life Support - CPR, within 30 Days and


Preferred

  • High School GED General Studies, upon hire or
  • High School Graduate General Studies, upon hire
  • First Aid, upon hire
