As a Primary Care Liaison RN, you will be an integral part of our primary care team, operating to deliver comprehensive, team-based patient care. You will foster a patient-centered Medical Home environment, collaborating closely with physicians and staff while also providing leadership to LVNs and MAs.
Every day you will utilize protocol-driven standing orders for a wide range of tasks, including evaluating lab data, providing patient education, performing phone triage, and assisting with medication modifications. You will actively coordinate care by facilitating diagnostic testing, ensuring timely follow-ups, and managing disease education, alongside contributing to process improvement and resource coordination.
To be successful in this role, you will expertly apply your RN scope of practice autonomously and collaboratively within the primary care setting. Your exceptional communication, organizational skills, and commitment to patient advocacy will be vital for effective care coordination, team leadership, and optimal patient outcomes.
