Job Summary and Responsibilities

As a RN Care Coordinator, you will be a central figure in patient care, seamlessly navigating the healthcare journey to achieve optimal outcomes and an exceptional patient experience.

Every day, you will strategically assess, plan, and facilitate comprehensive care across the continuum, expertly advocating for patients while collaborating with physicians, nursing, departments, insurers, and post-acute providers to ensure timely, high-quality transitions.

To be successful in this role, you will possess strong clinical acumen, exceptional communication and advocacy skills, and a strategic mindset, all driven by a passion for optimizing patient care across every touchpoint.
  • Functions as a clinical and leadership mentor and coach to new and existing staff
  • Assists with preparation and oversight of unit schedules.
  • Works with department management to balance schedules
  • Oversees and adapts the daily staffing plan to meet the department’s needs
  • Establishes patient assignments for a team of employees.
  • Participates in the peer interviewing process in the selection of unit staff.

Job Requirements

Required
  • Graduate of an accredited school of nursing, upon hire and
  • Registered Nurse: AR, upon hire and
  • Basic Life Support - CPR, upon hire

Preferred
  • Bachelors Other, upon hire or
  • Admission to a BSN program within one year, 12 - months or
  • obtaining a Bachelor degree within 2 years of assuming position, 24 - months
