Job Summary and Responsibilities

As our Wound Ostomy Nurse, you will connect our hospital directly to the community, delivering specialized in-home care that empowers patients to heal comfortably and effectively.

Every day you will perform advanced treatments like PICC line care, wound vacs, and infusions, while educating patients and families on complex conditions and treatments.

To be successful in this role you must be an exceptionally organized, independent professional, adept at managing complex schedules, patient care, and technology-driven documentation. Your deep commitment to patient engagement and advocacy will make you a vital defense against declining health, fostering a profound impact on individual well-being.

  • Conducts care assessments and interprets assessment data to determine care needs and constraints in meeting these needs.
  • Successfully completes system-based and hospital specific competencies.
  • Maintains accurate documentation of observations, treatments, teaching, medications, care given, patient response, and integrates into plan of care.
  • In collaboration with patients and families (as defined by the patient) the nurse is responsible for evaluating, planning, directing and providing wound/ostomy care to the patient. Implements a plan of care based upon established standards of care/practice, policies, and procedures, incorporating cultural, religious, psychosocial, physical, and age-related issues.
  • Possesses knowledge of medications, and administers safely and accurately.
  • Appropriately performs, delegates, and supervises routine treatments to provide wound/ostomy care services.

Job Requirements

Required:

  • BSN and Minimum two years of experience as an RN in an acute care facility. , upon hire
  • Registered Nurse: KY, upon hire and
  • Basic Life Support - CPR, upon hire and
  • Certified Ostomy Care Nurse, within 24 - months or
  • Certified Wound Care Nurse, within 24 - months or
  • Certified Wound Ostomy Continence Nurse, within 24 - months
