As our Social Worker, you will be a compassionate advocate, providing vital support and guidance to individuals and families facing complex challenges.
Every day, you will conduct psychosocial assessments, develop care plans, provide counseling and crisis intervention, and connect clients to resources. You'll advocate for clients and collaborate with multidisciplinary teams to ensure holistic support and promote well-being.

- Conduct and document psychosocial assessment of patients
- Share responsibility in identifying appropriate decision makers if patient is without capacity, educating all in patients’ rights and encouraging participation in decision‐making and care
- Provide liaison services and referrals to hospital resources and community agencies, including medication assistance programs
- Identifies mental health needs of patients and Provides counseling and referral as indicated
- Collaborates with interdisciplinary healthcare team members to ensure continuity and coordination of patient care throughout the continuum and into the community by coordinating placement with home health agencies, long term acute hospitals (LTACs), skilled nursing facilities (SNF) and assisted living facilities, nursing home placement, acute rehab, psych placement, and custodial care facilitation
- Serves as a resource to the patient, family, and healthcare tem regarding emotional and behavioral issues and refers to community services available to adolescent, adults and elders
