Job Summary and Responsibilities

Under the direct supervision of the agency Director, with indirect supervision from the Chaplain Coordinator, the Hospice Spiritual Care Provider is responsible for providing spiritual/bereavement care to hospice clients, families, staff and volunteers.

 

Essential Functions:

 

Documents assessment, planning and coordination of Spiritual Care of the patient/family:

 • Spiritual Assessment as documented in the electronic medical record.

 • Patient/family care plan reflecting individualized goals and interventions.

 • Spiritual assessment completed within five calendar days of admit.

 

Meets Spiritual care needs of patient and family:

 • Visits made per frequency order and notes reflect ongoing assessment of spiritual needs.

 • Increased visits/contacts as patient status declines.

 • Provide spiritual/emotional support to patients and families.

 • Coordinates spiritual care with community clergy as directed by the plan of care.

 

Coordinates agency Bereavement Services:

 • Completes initial assessment within 4-6 weeks following request for bereavement follow-up

 • Develops bereavement plan of care; documents bereavement care in the electronic medical record.

 • Provides grief/loss counseling to bereaved, as indicated in plan of care

 • Coordinates bereavement mailings

 • Conducts agency grief support groups

 • Coordinates hospice memorial events and community grief education events

 

 

Facilitates and/or coordinates in-services, presentations and special events for the Hospice program:

 • Assists in providing education for Hospice volunteers

 • Coordinates and assists in development of special events

 • Provides spiritual education to hospice team

 • Actively participates in committees working on Hospice related issues and needs.

 

 

Job Requirements

Education appropriate for spiritual care provider

Experience providing bereavement support

Completion of pastoral care and bereavement program training

Working knowledge of spiritual assessment and intervention to support patient and family

Excellent verbal and written communication skills

Driver's License

Auto Insurance

Basic Life Support - CPR
