Job Summary and Responsibilities

This position provides nursing care across the continuum of care services for patients that present with arrhythmia/atrial fibrillation conditions to facilitate appropriate advanced assessment and procedures. The incumbent will formulate a multi-disciplinary team approach to offer therapies and provide assessment to assure correct patient selection and therapy decisions. The incumbent designs and implements this new MGH program's services and activities and provides maintenance of all aspects of the program in collaboration with the physicians and other multi-disciplinary team members. This position may be involved in research clinical trials, patient and professional educational programs and continued patient follow-up. The role of this position will evolve over time to meet the needs of the patients and the program. The incumbent is accountable for maintaining quality standards for the responsive service and assures professional documentation; and maintains current and accurate policies, procedures, and practices; all in a manner that is consistent with the Mission and Philosophy of the Sisters of Mercy.

  • Documents assessments, evaluations, recommendations and other pertinent information in the patient's medical records.
  • Navigates the intake clinical referral functions, screening, additional patient testing requirements, consult appointments and hospital based procedures. Works collaboratively with Arrhythmia/Atrial Fibrillation Team to facilitate communication with EP physicians, CV surgeons, and cardiologists, other specialists as needed, and referring physicians regarding patient eligibility and clinical status.
  • In the acute care environment, collaborates with the nursing staff and educators, case managers, and outpatient services to ensure prompt movement of patients along the continuum of care. In conjunction with the Physician team, support professional education to clinical team members in the advanced techniques and care of patients receiving specialized interventions.
  • Prepares the patient and family for the procedure. Provides support to the physician within scope of practice in follow-up care per pre-determined clinic intervals, documents patient status and communicates ongoing patient condition to physicians during follow-up care.
  • Collaborates with an interdisciplinary cardiovascular health care team and Quality Management in delivery and evaluation of ongoing care programs.
  • Facilitates ongoing program development to allow the Advanced Heart Disease Clinic to meet the ever-changing business climate of Dignity Health Heart and Vascular Institute of Greater Sacramento. In conjunction with the Arrhythmia/Atrial Fibrillation Team, physicians and program director, development may include but is not limited to: clinical care forms, patient flow charts, patient and physician professional communication tools, clinical care protocols, registry, patient data tracking methodology (using 4D), program goals and outcomes reporting.

Job Requirements

Minimum Qualifications:

  • Current California RN license
  • Current ACLS and BLS through American Heart Association (AHA).
  • Completion of an Associate's degree in nursing or other related field.
  • At least three (3) years experience caring for cardiovascular surgical/ interventional patients

Preferred Qualifications:

  • ICU and/or EP/cath lab
  • Strong computer skills using Microsoft Word Excel Power Point.
  • Database experience preferred.
