Job Summary and Responsibilities

The Clinical Pharmacist provides comprehensive pharmaceutical care services to patients, working collaboratively with physicians, nurses, and other healthcare professionals to optimize medication therapy and improve patient outcomes. The Clinical Pharmacist is responsible for medication order review, pharmacokinetic monitoring, drug information provision, patient education, and participation in clinical initiatives.



  • Review and verify medication orders for appropriateness, safety, and efficacy, considering patient-specific factors such as age, weight, renal function, and allergies. 
  • Monitor serum drug concentrations and adjust dosages as needed to achieve therapeutic goals and minimize toxicity.
  • Provide accurate and timely drug information to physicians, nurses, and other healthcare professionals. 
  • Educate patients on their medications, including proper administration, potential side effects, and drug interactions. 
  • Conduct comprehensive medication reviews and provide MTM services to patients with chronic diseases. 
  • Participate in antimicrobial stewardship programs to promote the appropriate use of antibiotics and prevent antimicrobial resistance. 
  • Monitor for and report adverse drug reactions.

Job Requirements

  • Bachelor of Science in Pharmacy or Doctor of Pharmacy degree
  • Active State Pharmacist License in good standing in the state in which the employee works (RD PHARM)
  • AHA BLS 
  • Fire Safety card within 90 days of hire.

 

Preferred:

  • AHA  ACLS
