Job Summary and Responsibilities

Under direct supervision the incumbent identifies and evaluates potential clients as total person with focus on health limitations which impede independent living. The incumbent will certify level of care for the clients, provide medical information and training to care management staff, participate in California Community Transitions program and assist the development of organizational programs in response to changing health care services

  • Conducts in-depth assessments/reassessments covering medical, health, and rehabilitation concerns. Performs physical assessments as necessary and interpret clinical findings
  • Certifies level of care determinations
  • Works closely with physicians and other health care professionals. Consults with team members and participates in team care management activities
  • Develops care plans, implements and monitors services, consults with other team members, completes case recording and reporting. Maintains case load
  • Participates with CCT providing assessments, feedback and planning for patients
  • Assists with the development of organizational programs to create new services

Job Requirements

Minimum qualifications

  • Three (3) years experience two (2) of which are in public health
  • Associate of Science degree in nursing
  • Valid current California license to practice nursing

 

Preferred Qualifications 

  • Geriatrics experience 
  • Bachelors degree in nursing
