Where You’ll Work

CHI St. Alexius Health Garrison includes the services of a 22-bed critical access hospital, a 26-bed skilled nursing facility, an attached rural health clinic, and an Emergency Department that is available 24 hours a day. We offer many outpatient services, including physical therapy, radiology services, laboratory services, cardiac rehabilitation, and IV treatments. Since opening in 1952, CHI St. Alexius Health Garrison has been dedicated to serving the residents of Garrison and the surrounding rural communities. The hospital is equipped with 4 emergency rooms, on-site lab, radiology, CT and physical therapy services. Our patients have access to 24/7 emergency care, swing-bed program, and acute care services.

Job Summary and Responsibilities

As our RN Health Coach you will help patients, physicians, advanced practice providers, staff, and other professionals coordinate patient care across the continuum so they can achieve improved health outcomes and navigate the healthcare system effectively. Every day you will manage disease registries by identifying and following up with patients not meeting clinical goals based on evidenced-based guidelines, and serve as a liaison with specialty practices, in-patient facilities, and community resources. You will be expected to work autonomously, use education, goal setting, action planning, problem-solving, and motivational interviewing to support health behavioral change, and contribute leadership to the healthcare team. To be successful in this role, you must demonstrate excellent communication skills, independence, assertiveness, and critical thinking when working with patients and co-workers, possess strong organizational skills, and practice nursing in accordance with the North Dakota Nurse Practice Act and the limitations of your scope of practice.

  • Patient Assessment & Advocacy: Provides comprehensive patient support, assessing emotional/cognitive status and learning readiness, while also acting as a patient advocate within the healthcare system
  • Care Coordination & Collaboration: Collaborates with primary care providers, interdisciplinary teams, and community resources to ensure continuous, high-quality care and address changing patient needs
  • Chronic Disease Management: Effectively utilizes the nursing process and disease registries for preventive maintenance and chronic disease management, identifying and addressing gaps in care
  • Patient & Family Education: Delivers effective education to patients and their families, empowering them to meet disease-specific goals
  • Performance & Quality Improvement: Collaborates with internal leaders to support, implement, review, share, and submit performance metrics to enhance quality outcomes
  • Communication & Team Leadership: Ensures safe patient transitions through effective communication, and demonstrates accountability by coaching, mentoring, and training applicable staff members

 

Job Requirements

Required

  • Graduate of a Registered Nurse program
  • Current license as a Registered Nurse in the state of ND

 

Preferred  

  • Preferred 2-3 years hospital or clinic setting
  • RN Health Coach certification or completion within 18 months of hire
