Job Summary and Responsibilities

As our Community Health Worker, you will be a vital link between individuals and essential community resources, actively working to reduce health disparities and improve access to support services.

Every day, you will proactively engage with clients, assessing their needs and identifying pathways to care and support. You'll connect individuals to relevant health, social, educational, and employment resources, providing guidance and practical assistance to overcome logistical barriers.

To be successful in this role, you will possess a profound understanding of diverse community needs, exceptional problem-solving abilities, empathy, and resilience. Your dedication to empowering clients and removing barriers to care will be paramount.
  • Responsible for conducting outreach and identifying innovative solutions to reach community members in a safe and accessible manner.
  • Develops and maintains relationships through outreach, engagement and culturally competent health education activities in the community, while securing site locations for outreach work and health education classes and/or events.
  • Establishes and keeps current a resource file/directory of information regarding available programs, services, contact persons and avenues of access in the community.
  • Develops and distributes outreach and educational materials and other marketing aids to promote health education activities while actively maintaining a presence in the community.
  • Implements and facilitates health education events, workshops, classes, and public presentations covering approved topics and/or evidence-based programs, either in-person and/or through video conferencing methods.
  • Responsible for follow-up activities to recommend, refer and provide linkage to appropriate services while communicating with sensitivity to clients from diverse language and cultural backgrounds.

  • Associates Other Two years of college education or equivalent work experience related to field, upon hire and

Preferred
  • Bachelors Other Bachelor's degree in health education/community health/ public health and Experience conducting outreach andsupportive service delivery., upon hire
