As our Nurse Manager of Care Coordination, at Mercy San Juan Medical Center, you will help our care coordination team (35 FTEs including RNs and CCAs) efficiently and compliantly execute the organization's Care Coordination goals and key performance metrics so they can constantly improve patient experiences, reach out to our community, and deliver quality care.
Every day you will oversee the daily operations of the department, encompassing staff performance management, reviewing or gathering data to promote performance improvement, and participating in or facilitating meetings and projects related to departmental functions. You will be expected to communicate the organization’s goals to your team while also advocating for them by bringing their issues, requests, or new ideas to leadership.
To be successful in this role you must possess strong leadership and communication skills, an understanding of the factors contributing to quality patient care, knowledge of various managed care service principles and techniques, and a solid grasp of federal, state, and local healthcare-related laws and regulations.
