Colorado Health Neighborhoods (CHN) is the region’s largest physician-led, clinically integrated network with more than 5,000 primary and specialty care physicians and other health professionals who care for more than 230,000 people across Colorado. Formed in 2011 as part of our partnership with our population health group, CHN is leading the charge to transform health care delivery from a reactive, fee-for-service model to a pro-active, value-based model centered on the patient.
