Job Summary and Responsibilities

As our Social Worker, you will be a compassionate advocate, providing vital support and guidance to individuals and families facing complex challenges.

Every day, you will conduct psychosocial assessments, develop care plans, provide counseling and crisis intervention, and connect clients to resources. You'll advocate for clients and collaborate with multidisciplinary teams to ensure holistic support and promote well-being.

To be successful in this role, you will possess exceptional interpersonal skills, empathy, strong knowledge of community resources, and proven crisis management abilities, fostering positive change and client empowerment.

  • Conducts in-depth assessments/reassessments covering psycho-social, rehabilitation and environmental concerns
  • Refers clients to special consultants to provide detailed evaluations and recommendations to improve the client's functional level
  • Develops the care plan, implement the services and conduct follow-up and monitoring of the plan
  • Identifies and develops support systems for the client
  • Consults with the Health Practitioner
  • Performs case recording and reporting

Job Requirements

Minimum Qualifications:

  • Bachelors Other in social work,psychology, counseling, rehabilitation,gerontology or sociology, One (1) year experience working with theelderly and individuals with disabilities or
  • Two years of experience providing social service in one or more of the fields of aging health and long term care services required
  • Ability to relate to clients and families
  • Excellent communication and presentation skills
