Job Summary and Responsibilities

The incumbent assists patients and their families with ADLs and IADLs in their place of residence and functions as a member of the Hospice Interdisciplinary Team.

 

As our Home Care Aide, you will provide essential personalized support and companionship to clients in their homes, enhancing their overall quality of life and fostering independence according to individualized care plans. Every day you will assist clients with activities of daily living (ADLs) such as personal care, meal preparation, medication reminders, light housekeeping, and transportation to appointments. You will meticulously document services provided and proactively communicate any changes in the client's condition to the appropriate supervisor. To be successful in this role, you will possess a compassionate and patient demeanor, excellent communication and interpersonal skills, and a strong commitment to client dignity and well-being. Your ability to adapt to varying client needs, maintain a safe environment, and adhere to care plans with attention to detail is essential.

Job Requirements

  • Current certification by the California Department of Health Services as a HomeHealth Aide.
  • One (1) year in the last 24 months, this must be paid experience.
  • CHHA in a health care facility or HomeHealth setting.
  • Home Health Aide: CA.
  • Basic Life Support from the American Heart Association.
  • Driver's License: CA.
  • Auto Insurance.
