In addition to 24/7, Level III emergency care at St. Anthony-Summit Hospital in Frisco, we have three emergency mountain clinics at the base of the Breckenridge, Copper Mountain and Keystone resorts, as well as urgent care and primary care available from CMG High Country Healthcare clinics in Frisco, Breckenridge and Silverthorne.

Job Summary and Responsibilities

As our Diabetes Educator, you will develop curriculum and teaching materials, coordinate and lead instruction for our American Association of Diabetes Educators (AADE) certified diabetes program, and collaborate with a multidisciplinary team to provide quality patient care.

Every day you will procure resources for uninsured/underinsured patients, teach diabetes education skills to Type 1 and Type 2 diabetic patients in both class and individual sessions, and maintain optimal program development, policies, marketing, and data outcomes.

To be successful in this role, you will possess expertise in diabetes education, strong program development and teaching skills, and the proven ability to lead, innovate, and drive change. Your commitment to maintaining AADE certification and ensuring program excellence will significantly improve patient care outcomes.

 

  • Develops and provides individualized diabetes selfmanagement/training by incorporating evidence based research, principles of adult learning, AADE selfcare behavior theory to empower the patient, family with diabetes to manage the chronic disease.
  • Maintains certification requirements of the program by following AADE policies and submitting annual outcomes assessment.
  • Leads diabetes advisory group meetings comprised of a multidisciplinary team that oversees quality of the program.
  • Documents assessment, ongoing status, interventions, patient response and outcomes in an accurate, timely manner.
  • Monitors referrals and patient enrollment process with multidisciplinary staff from centralized scheduling and health benefits staff.
  • Cooperates with insurance preauthorization and documentation requirements for compensation. Designs curriculum, class schedule diabetes support group events, and marketing for the program.

 

$400/month Summit County housing stipend available for full time residents of Summit, Park, Eagle, Grand, Clear Creek, Lake, and Chaffee Counties. Must work at least 80% physically on-site at a CommonSpirit Mountain Region site in Summit County and full time or part time status (PRN ineligible). More details available upon offer.

Job Requirements

Required

  • Associate's Degree
  • Three (3) years of experience in patient education
  • One (1) year experience in diabetic education
  • Certified Diabetic Educator
  • Registered Nurse Registered Dietitian or Registered Nurse in the state of employment

Preferred

  • Bachelor's Degree
