Job Summary and Responsibilities

The incumbent is a technical caregiver who assumes responsibility and accountability for Hospice patients and provides care to these patients via therapeutic use of self, the environment/instrumentation, and other health care team members, under the oversight of a registered nurse. Incumbent shares in the rotation of 24-hour, seven day-a-week call coverage to patients/families as necessary.

 

  • Utilizes the nursing process for planning and delivery of direct and indirect patient care. Evaluates effectiveness
    of nursing interventions and updates Plan of Care as appropriate to patient condition/need.
  • Provides personal care assistance to patients including: bathing, skin care, dressing, eating, elimination, transportation, and ambulating
  • Performs documentation in patient records that is timely, accurate, and concise
  • Provides appropriate support at time of death
  • Performs efficiently in emergency situations, following established protocols, appropriate communication and
    documentation
  • Communicates with Interdisciplinary Team members regarding patient status and changes to Plan of Care per
    Hospice policy
  • Assumes responsibility for professional growth by taking advantage of the available opportunities.
  • Participates in peer review and staff development, which includes unit and quality process improvement activities

 

Please be advised that this location requires quarterly American Heart Association (AHA) Resuscitation Quality Improvement (RQI) competency certification post-hire. This ongoing competency is required through resources internal to the facility; outside RQI competency resources are not acceptable for CommonSpirit Health compliance requirements.

Job Requirements

Required

  • Graduate of a BRN approved school of vocational nursing
  • Minimum of 1year acute experience within previous 5 years in Acute Care ,Hospice or Home Health.
  • Licensed Vocational Nurse: CA (LVN:CA)  
  • Basic Life Support - BLS through an American Heart Association provider
  • Driver's License: CA (AUTO:CA) 
  • Auto Insurance (AUTO-INS)

Preferred

  • Hospice LVN experience
