Job Summary and Responsibilities

Responsible for procuring, manufacturing, compounding, dispensing, distributing, storing and administration of drugs, biologicals, and chemicals according to standard practices of the professional, State and Federal laws and regulations, and departmental policies and procedures. Responsible for provision of drug information to other health professionals, patients and their families, and the public.

  • Reviews all orders for appropriate drug therapy, proper route dose, drug interactions, appropriateness for age and


    contraindications. Always obtains patient's allergies to medications and checks for potential allergies prior to dispensing any medications. Checks patient's profiles for interactions. Participates in physician intervention program and documents interventions.
  • Reviews MIC reports daily checking report against antibiotic therapy. If antibiotic not appropriate will contact physician.


    Always documents actions taken on report.
  • Performs daily assessment of drug therapy appropriateness for patients with renal impairment and those receiving narrow


    therapeutic range medications.
  • Detects and reports on ADRs. Completes ADR report form with pertinent and complete information. Reports on at least


    two ADRs a month (each full time pharmacist).
  • Researches and provides consultation to physicians and nurses relating to drug therapy, including incompatibilities of IV


    solutions and age-specific doses and dosage forms.
  • Assesses the patient retrieving or ordering all necessary baseline data. Able to plan therapy and orders all medications


    and appropriate labs, documenting daily on pharmacy sheets all relevant data. Always follows established guidelines taking into account all patient variables. Performs thorough daily assessment, monitoring and documentation of patients on pharmacy protocol. Writes in patient's medical chart as necessary.

Required

  • 1-3 years Two years hospital experience withparticipation in a clinical program if no residency. , upon hire 
  • 1-3 years Two years managing andoverseeing pharmacy technicians., upon hire
  • Basic Life Support - CPR, upon hire 
  • Pharmacist: CA, upon hire
