Job Summary and Responsibilities

As our Dietitian, you will integrate advanced knowledge of nutrition science, medical nutrition therapy, chronic disease management, and behavioral health to deliver evidence-based nutrition counseling and education to ambulatory patients and community members. 

 

Every day, you will apply motivational interviewing techniques and adult learning principles to support sustainable behavior change across individual and group settings. The Dietitian provides telehealth-based counseling in addition to in-person services, participates in interdisciplinary care team collaboration, and contributes to the planning and implementation of Education Services programs within the Community Health Education department.


To be successful in this role, you will possess strong communication and relationship building skills, as well as the ability to apply adult learning principles.

 

- Provides individual and group nutrition counseling to patients, including via telehealth platforms.
- Collaborates with interdisciplinary care teams, including physicians, nurses, and social workers, to ensure coordinated and comprehensive patient care.
- Conducts comprehensive nutrition assessments, including chronic disease management (e.g., diabetes, cardiovascular disease, obesity), and selects appropriate counseling techniques and processes.
- Plans, develops, and delivers curriculum for health education programs aligned with evidence-based practice.
- Documents all patient encounters, assessments, care plans, and outcomes accurately and timely in the Electronic Health Record (EHR).
- Screens patients for food insecurity and social determinants of health; facilitates referrals to appropriate community resources as needed.

Job Requirements

Minimum Qualifications:

 

- 1 year of clinical dietetics experience required.
- Bachelors degree in Dietetics or Clinical Nutrition.
- Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) license.
- Experience with telehealth platforms and delivery of remote nutrition counseling.
- Knowledge of age and gender-specific nutritional characteristics.
- Ability to apply adult learning principles in educational settings.
- Proficiency in motivational interviewing techniques to support behavior change.
- Knowledge of chronic disease management through medical nutrition therapy (e.g., diabetes, cardiovascular disease, obesity).

- Knowledge of social determinants of health and food insecurity screening.
- Proficiency in Electronic Health Record (EHR) documentation and navigation.
- Effective listening and verbal communication skills. Ability to communicate effectively with individuals as well as groups.
- Ability to independently problem solve and/or bring problems to the team for feedback.
- Cultural humility and ability to provide equitable, culturally responsive care.


Preferred Qualifications:


- Experience in an ambulatory care setting preferred
- Bilingual ability (preferred, based on community needs).
