  • Accurately document patient information, symptoms, and care provided in electronic health records (EHR) to ensure comprehensive medical record maintenance.
  • Collaborate closely with primary care providers (Physicians, NPs, PAs) to ensure seamless patient flow and effective communication surrounding patient care plans.
  • Participate in patient education, explaining treatment plans, medication instructions, and preventive health measures.
  • Attend/conduct in-service presentations and seminars to facilitate personal and professional growth, enhancing your skills within the primary care setting.
  • Demonstrate personal accountability for relationship-based care, upholding the organizational mission, and core values that are fundamental to our patient-centered approach.
