Job Summary and Responsibilities

As our Case Management Manager, you will help our team optimize the daily operations of our case management functions so they can deliver efficient, compliant, and high-quality patient care.

Every day you will oversee and guide our case management team. You will be expected to drive performance improvement, manage staff, and ensure seamless care coordination, discharge planning, utilization review, and social work interventions in accordance with all relevant policies and regulations.

To be successful in this role, you must possess strong leadership skills, a deep understanding of case management principles, and the ability to effectively manage and develop a team in a complex healthcare environment.

  • Strategic Leadership: Defines clear roles, sets goals, leads initiatives, and adjusts plans as necessary, always aligning with the organization's mission, vision, and values.
  • Patient/Customer Focus: Ruthlessly prioritizes the patient perspective in actions and decisions, implementing service practices that meet both patient and organizational needs.
  • Empowerment & Trust Building: Effectively shares authority and responsibility, delegating to develop team members, and interacting in a way that instills confidence in intentions and the organization.
  • Operational Excellence & Regulatory Compliance: Possesses strong technical and functional competence, including a deep understanding of operational requirements and the ability to ensure compliance with the healthcare regulatory environment.
  • Conflict Resolution & Performance Improvement: Adept at anticipating, recognizing, and dealing effectively with conflicts, while also driving clinical performance improvement to ensure quality patient care.

Job Requirements

Required

  •  A minimum of three (3) years in a hospital-based Clinical Social Work role.
  • Master's Degree in Social Work from a university or college accredited by the Council of Social Work Education.
  • License Clinical Social Worker
