Job Summary and Responsibilities

As our Social Worker you will proactively assess, develop, and implement comprehensive plans of care in collaboration with patients, families, and an interdisciplinary team. 

 

Essential Functions

  • Completes psychosocial assessments, interviews patients, relatives, friends or significant others to obtain information about the patient’s personal, social and emotional history.
  • Provides therapeutic care to patients to assist them in accomplishing treatment goals using interventions and techniques that are appropriate to the age and developmental level of the patient.
  • Coordinates treatment with community agencies such as social services, schools and probation.
  • Maintains social service records of each patient according to hospital and departmental program standards. Maintains statistics and turns in reports in a timely manner.
  • Communicates with crisis team for mental health emergencies. Coordinates outpatient treatment following inpatient hospitalization
  • Assists in revision of policies and provide input to the departmental budget by identifying departmental supply and equipment needs to meet departmental goals.
  • Adherence to Social Work Code of Ethics

Job Requirements

Required

Bachelor's Degree in Social Work

Licensed Clinical Social Worker: MN (LCSW:MN)

 

Preferred

Master's Degree in Social Work
