Location: RN Clinic Coordinator for Structural Heart CUMC-Bergan 77th & Mercy
Schedule: Monday-Friday 730a-430p, No Weekends, No Holidays & No Call
In collaboration with the cardiologists and surgeons the RN Clinic Coordinator is responsible for providing and facilitating activities and services regarding care for patients with Structural Heart disease and disorders in need of interventional cardiology, such as valvular heart disease. Responsible for ensuring all patients who are navigated receive quality and comprehensive services. Coordinates patient care throughout the continuum in collaboration with the multidisciplinary team. Screens and reviews tests of complex patients with cardiovascular disease undergoing transcatheter aortic valve replacement (TAVR), transcatheter mitral edge to edge repair, transcatheter tricuspid therapies, left atrial appendage closure devices, PFO closure, Atrial Septal Defect Closure, Perivalvular Leak Repair, and Alcohol Septal Ablation. Responsible for evaluating and managing studies for suitability and interacting with the multidisciplinary team and industry. Coordinate patient care and evaluation, design and align resources to support program initiatives, program development, develop and maintain relationships with internal and external referring physicians, and development of a program databases.
Coordinates the referral, screening, additional patient testing requirements, consult appointments and hospital based procedures. Facilitations communication with cardiologists and cardiovascular surgeons, including referring physicians regarding patient eligibility and clinical status.
Serves as a point of contact to all patients from diagnosis, treatment and recovery period.
Facilitates the progression of the valve procedure patient and assist with the preparing the patient and family. Provides follow up care per pre-determined clinic intervals, documents patient status and communications ongoing patient condition to physicians and APPs during follow up care.
Collaborates with a multidisciplinary cardiovascular cardiovascular health team in planning, implementing ongoing care for patients including running a monthly conference for patient discussion and shared decision making processes.
Works closely with vendors involved in the day to day management and product selection for this patient population.
Serves as a clinical resource for identification, tracking and treatments for Structural Heart/valve patients; orders diagnostic studies and/or clinical interventions when appropriate under the direction of APP or physician leaders, as well as measures patient-specific and aggregate outcomes; tracks new technology trends, research findings, treatment models, etc.
Facilitates referrals for Structural Heart procedures. Coordinates appropriate testing and collates results for review. Utilizes appropriate software for coordination of images/studies.
Performs education related to Structural Heart to patients, family and staff. Ensures coordination of care.
