With guidance from the local market leadership, the RN Care Coordinator works collaboratively with physicians, staff and other health care professionals within his/her clinically setting to maintain and improve quality and sustainability within the local market network, this work includes:

AA degree in Nursing is required
BSN preferred
2 years relevant experience or an advanced degree required, i.e. Masters Degree
State issued license as a Registered Nurse
Physical Demand Level: Light
The hybrid role reports to our corporate office a few times a year. It also has to go to the provider's office on occasion. The rest of the time, the role is remote.
