As our Ventricular Assist Coordinator RN, you will provide specialized nursing care centered on the medical and surgical care of patients with advanced heart failure who are potential candidates for mechanical circulatory support, all patients who are on advanced forms of mechanical circulatory support, and/or are being bridged to a heart transplant.
Every day, you will play a critical role in managing these complex patients, from initial assessment and education through ongoing support and coordination of their advanced therapies. This involves meticulous care planning, patient and family education, collaboration with multidisciplinary teams, and vigilant monitoring to ensure optimal outcomes for highly vulnerable individuals.
To be successful in your role, you will strategically coordinate comprehensive nursing care for patients with advanced heart failure, particularly those on or being considered for mechanical circulatory support or heart transplant. You will demonstrate exceptional clinical expertise, provide specialized support, and manage complex care needs with precision, ensuring optimal outcomes for a critically ill patient population throughout their unique continuum of care.
