Job Summary and Responsibilities

As our Cancer Navigator, you will assess the needs of cancer patients, families, caregivers, and the community throughout their cancer journey.

Every day you will provide education, support, and direction before, during, and after diagnosis, collaborating with treatment teams to address physical, psychological, spiritual, and social needs by mitigating barriers to care.

To be successful, you will enhance the patient experience by connecting them to appropriate resources, demonstrating compassionate communication, problem-solving skills, and a commitment to holistic support.

  • Provide comprehensive patient and family support: Coordinate care, educate on disease and treatment, and offer emotional and practical support throughout the care continuum.
  • Conduct clinical assessments, manage symptoms, and triage needs: Perform comprehensive nursing assessments, evaluate presenting symptoms, assess pain, and manage treatment-related symptoms. The role also involves skillfully triaging patient inquiries and clinical needs, reporting key findings and interventions to providers promptly.
  • Facilitate timely access and coordination or care: Ensure prompt access to diagnostics and treatment, coordinate appointments efficiently, and integrate care with the multidisciplinary team.
  • Identify and address barriers to care: Assess for physical, psychosocial, financial, and logistical barriers, making appropriate referrals to resources.
  • Promote patient and family-centered decision-making: Educate and empower patients and families to make informed choices, including advocacy and understanding of treatment options.
  • Communicate effectively with all stakeholders: Maintain clear and professional communication with patients, caregivers, families, and the entire healthcare team, including managing phone inquiries.

Job Requirements

Required

  • Bachelors of Science Nursing
  • Three (3) years of experience as an RN in an inpatient or outpatient setting
  • Registered Nurse: AZ
  • American Heart Association Basic Life Support - CPR
