Job Summary and Responsibilities

 

As our Per Diem Night On-Call Hospice Registered Nurse (RN), you will help bring compassionate care, treatment, and support directly to the homes of hospice patients and their families, so the family can care for their seriously and life-threatened loved one at home, especially when facing a prognosis of six months or less.

Every day, you will deliver direct patient care and expertly coordinate with a diverse team, including physicians, nurses, social workers, counselors (including dietary), chaplains, home health aides, and volunteers. You will 'll be expected to meet discipline-specific productivity standards, demonstrate clinical competency through observation, and ensure strict compliance with all agency, state, and federal regulations.

To be successful in this role, you must be a California-licensed driver with valid car insurance. This enables you to be on call for any patient needs that arise after hours, providing support when the interdisciplinary team is off, and could include admissions to bring patients onto hospice services when needed. 

  • Meets the discipline-specific productivity standard.
  • Demonstrates competency in the care of patients during field observation by the clinical supervisor. 
  • Completes and submits documentation following established guidelines, as measured by defined evaluation processes and tools. 
  • Documents follow-up of identified problems, reviews and documents clinical progress toward goals and physician orders are written according to policy. 
  • Demonstrates coordination of care by appropriate communication with other disciplines and documents that communication, as measured by defined evaluation/audit processes. (doc. communications, IDT notes, progress notes etc.). 
  • Demonstrates compliance with agency processes to meet State regulatory and Federal Conditions of Participation requirements as measured by defined evaluation processes (i.e. U.R review, Patient Care Planning, Admit, IDT processes etc.)

 

Job Requirements

Required

  • One (1) year Hospice nursing (LVN or RN) experience in the past five (5) years
  • Active Registered Nurse (RN) license in the State of California
  • Basic Life Support (BLS) certification - American Heart Association
  • Current California Driver's License, Auto Insurance and access to an operating motor vehicle.

 

Preferred 

  • Bachelors Of Science in Nursing 
  • Public Health Nurse (PHN) certificate
  • Certified Hospice and Palliative Nurse (CHPN) certificate
