Job Summary and Responsibilities

As our Social Worker, you will be a compassionate advocate, providing vital support and guidance to individuals and families facing complex challenges within the healthcare setting. Your expertise in navigating social, emotional, and practical issues will be crucial to promoting overall well-being and successful patient outcomes.

 

Every day, you will conduct psychosocial assessments, develop individualized care plans, provide counseling and crisis intervention, and adeptly connect clients to essential community resources. You'll be a strong advocate for clients' needs, collaborating seamlessly with multidisciplinary teams to ensure holistic support and promote their well-being throughout their healthcare journey.

 

To be successful in this role, you will possess exceptional interpersonal skills, deep empathy, proficient knowledge of community resources and support systems, and proven crisis management abilities, consistently fostering positive change and client empowerment.

 

Responsibilities:

  • Conducting comprehensive psychosocial assessments to identify patient and family needs, strengths, and challenges.
  • Developing and implementing individualized care plans that integrate medical, social, and emotional needs, working collaboratively with patients, families, and the healthcare team.
  • Providing supportive counseling, crisis intervention, and emotional support to patients and families coping with illness, trauma, loss, and other life changes.
  • Facilitating access to appropriate community resources, services, and support networks, including discharge planning, long-term care, financial assistance, and mental health services.
  • Advocating on behalf of patients and families to address barriers to care, ensure access to necessary services, and protect their rights and dignity.
  • Documenting all social work interventions, assessments, and care plans accurately and timely in the electronic health record, adhering to professional standards and hospital policies.

Job Requirements

Required

  • Bachelor's Degree in Social Work
  • Licensed Clinical Social Worker: ND (LCSW:ND)

 

Preferred:

  • Master's Degree in Social Work
