Job Summary and Responsibilities

As a Navigator RN, you will provide expert nursing guidance and support to patients as they navigate complex healthcare journeys.

Every day you will assess patient needs, educate on treatment options, coordinate care across multiple providers, and facilitate access to resources, acting as a central point of contact.

To be successful, you will demonstrate strong clinical knowledge, exceptional communication and problem-solving skills, and a compassionate, patient-centered approach to ensure seamless and informed care.

 

  • Coordinate patient care across the continuum from diagnosis through treatment, follow-up, and transition from care, ensuring timely, seamless coordination of services.
  • Assess patient and family needs, provide education on diagnoses and treatment options, and offer ongoing clinical, emotional, and psychosocial support throughout the care journey.
  • Collaborate with physicians, multidisciplinary teams, and community resources to develop individualized care plans, facilitate referrals, and address medical, social, and financial barriers to care.
  • Advocate for patients and families by promoting informed decision-making, coordinating diagnostic testing and follow-up care, and ensuring continuity of care.
  • Monitor patient progress, document assessments, education, and interventions, and promote adherence to evidence-based care plans and clinical guidelines.
  • Support quality improvement and departmental initiatives through accurate documentation, data collection, outcomes tracking, participation in interdisciplinary teams, and coordination of clinical research activities as assigned.

Job Requirements

Required

  • Bachelors Of Science Science or other related discipline and three years of related professional nursing work experience in the clinical focus area that demonstrates the attainment of the requisite job knowledge, skills and abilities, upon hire and
  • Associates Other or Additional equivalent qualifying work experience that demonstrates attainment of the requisite job knowledge/abilities may be substituted for two years of the degree requirement, upon hire and
  • Registered Nurse: WA, upon hire and
  • Basic Life Support - CPR, upon hire


Preferred

  • Masters Of Science Nursing or related field and Previous navigator work experience, upon hire
