Job Summary and Responsibilities

As our Social Worker, you will be a compassionate advocate, providing vital support and guidance to individuals and families facing complex challenges.

Every day, you will conduct psychosocial assessments, develop care plans, provide counseling and crisis intervention, and connect clients to resources. You'll advocate for clients and collaborate with multidisciplinary teams to ensure holistic support and promote well-being.

To be successful in this role, you will possess exceptional interpersonal skills, empathy, strong knowledge of community resources, and proven crisis management abilities, fostering positive change and client empowerment.

  • Performs psycho-emotional, social, and environmental assessments informing the development and implementation of the individual plan of care including direct interventions, counseling, referring to community resources and incorporating needs of family and/or caregiver. Utilizes standardized tools and conducts a home visit.
  • Daily monitoring of all interventions of participant attendance and modified in a timely manner based upon changes in participant situation as dictated by regulations.
  • Serves as key advocate, facilitates collaboration and serves as participant’s point of contact with clinical team, family and members of participant’s care team outside of Adult Day Health Center.
  • Provides concise, articulate and timely documentation of all pertinent information within required timeframe. Includes use of electronic medical records.
  • Responsible for all aspects of social work services for each participant as defined by Title 22. This includes but not limited to initial assessment, home visit, quarterly and daily documentation and discharge planning.
  • Facilitates individual and peer support addressing aging, grief, coping, substance abuse, behaviors and family issues.

Job Requirements

Required

  • Masters Other in social work,psychology or related field from anaccredited school and One year of demonstrated competence inneeds of frail, chronically ill, elderly anddisabled.
  • Basic Life Support - CPR


Preferred

  • Other coursework in gerontology, upon hire and
  • Prefer two years of demonstratedcompetence in needs of frail, chronicallyill, elderly and disabled.
