Bruceville Terrace provides skilled nursing, rehabilitative and complex medical services to our patients and residents using an interdisciplinary team approach. Over half of the facility is dedicated to skilled nursing/convalescent care, while the remaining beds are committed to meet the medical and rehabilitative needs of the short-term patient after a stay at the acute care hospital. Job Summary and Responsibilities

As our Licensed Vocational Nurse (LVN), you will provide vital direct patient care, significantly contributing to patient well-being and recovery as an integral part of our compassionate healthcare team.

Every day, you will assess patient conditions, administer medications, perform prescribed treatments, and meticulously document care. You will collaborate with RNs and physicians to implement individualized care plans, ensuring a seamless and high-quality patient experience.

To be successful in this role, you will demonstrate exceptional clinical skills, a strong commitment to patient safety, and excellent communication. Your compassionate demeanor, attention to detail, and ability to prioritize in a fast-paced environment are essential.

  • Accurately collects admission data and records information on Admission/Assessment/History Record within two hours of arrival on unit.
  • Delegates data collection appropriately.
  • Observes and reports to the Registered Nurse, patient's responses to therapeutic interventions, hospitalization and diagnosis, including physical and psychological response.
  • Collaborates with the interdisciplinary team in meeting identified needs by appropriate referrals.
  • Collaborates with the Registered Nurse regarding discharge planning.
  • Demonstrates individual competency in selected psychomotor skills and independently performs technical procedures such as demonstrated ability to carry out procedures, assist physicians with procedures, safely use equipment, and proficiency on HIS.

Job Requirements

Required

 

  • One (1) year of SNF/LTC care of the aged, acutely ill, chronically ill and patients in need of restorative care.
  • Knowledge in the principles of nursing care within the LVN scope of practice.
  • Ability to communicate proficiently both orally and in writing in the English language.
  • Current Licensed Vocational Nurse. CA
  • BLS certified by the American Heart Association.
