Job Summary and Responsibilities

As our Community Health Worker at CommonSpirit Medical Group - Family Medicine - Ringgold, you will be responsible for assisting patients and their support team to navigate and access community services, other resources, and adopt healthy behaviors.

Every day, you will support the healthcare system providers and staff through an integrated approach to care management and community outreach. You will provide social support, informal counseling, and advocate for the health needs of individuals and their community. Community outreach and collaboration with community agencies are essential to this role. You will also participate in the process of collecting and maintaining health-related social needs screening data.

To be successful in your role, you will strategically assist patients and their support teams in navigating and accessing community services, resources, and healthy behaviors, thereby enhancing overall well-being. You will demonstrate exceptional proficiency in providing social support, informal counseling, and advocacy for health needs, meticulously collecting social needs screening data, and fostering strong community outreach and collaboration to create an integrated approach to care management.

  • Establishes rapport, assesses patient’s level of health literacy and comprehension, provides information/guidance for an effective care transition and ensures that information is understood by the recipient
  • Responsible for building and maintaining a positive working relationship with Providers, including, but not limited to, interactions within Provider offices, communication over the phone, and through digital means such as email and fax
  • Acts as an advocate for the patient/family by locating appropriate community resources to address concerns and reduce risk of hospitalization
  • Monitors effectiveness of services provided by external resources and provides timely feedback to the provider/interdisciplinary team
  • Escalates any internal and external customer concerns to the interdisciplinary team and/or Supervisor
  • Acts as an advocate for the patient and family by locating appropriate community resources to address concerns and reduce risk of readmission

Job Requirements

Required

  • High School Graduate, upon hire or
  • High School GED, upon hire and
  • 1-3 years Patient care environment, upon hire or
  • 1-3 years Working in a health-related community outreach organization, upon hire and
  • Certified Medical Assistant AAMA, upon hire or
  • Licensed Practical Nurse: TN, upon hire


Preferred

  • Associate Of Arts, upon hire 
  • Bachelors Of Arts, upon hire 
  • Knowledge and ability to work with electronic medical records; knowledge of health care terminology
