Job Summary and Responsibilities

As our Social Worker, you will be a compassionate advocate, providing vital support and guidance to individuals and families facing complex challenges.

Every day, you will conduct psychosocial assessments, develop care plans, provide counseling and crisis intervention, and connect clients to resources. You'll advocate for clients and collaborate with multidisciplinary teams to ensure holistic support and promote well-being.

To be successful in this role, you will possess exceptional interpersonal skills, empathy, strong knowledge of community resources, and proven crisis management abilities, fostering positive change and client empowerment.

  • Assesses, develops, implements and monitors a comprehensive plan of care through an interdisciplinary team process in conjunction with the patient and family in internal and external settings.
  • Education patients, families, and medical staff regarding available resources and guidelines associated in accessing resources. Provide applications as needed for these resources.
  • Resolves situations as appropriate within scope of job. Handles complaints/concerns in a prompt and courteous manner; assess and directs to appropriate person to deal with concerns as needed; makes attempt to solve issues with involved parties.
  • Demonstrates tact, courtesy, sound judgment and a professional attitude when relating with patients, families, coworkers, physicians, and affiliated agencies.
  • Gives and receives feedback in a positive manner.
  • Communicates both verbally and in writing with physician and appropriate hospital staff regarding patient needs and progress of discharge plan.

We are offering a sign on bonus up to $20,000 to hires who meet the eligibility requirements.
