Job Summary and Responsibilities

As a Patient Care Technician (PCT), you will deliver essential direct patient care under the supervision of our clinical team. Leveraging your knowledge of basic patient care principles and medical terminology, you will perform a range of daily duties.

Every day you will actively assist patients with personal care, mobility, and feeding, monitoring vital signs, documenting observations, and maintaining a therapeutic and organized patient environment. You will be expected to work closely with the nursing team to prepare patients for procedures or transport.

To be successful in this role requires a genuine passion for patient well-being, diligence in executing tasks, keen observational skills regarding patient conditions, and a proactive approach to continuous learning and professional development.

  • Assists the nurse/physician during examinations, treatments and procedures as requested. Retrieves necessary equipment and supplies. Assists in patient/equipment preparation. Performs follow-up actions as directed. Documents clinically relevant findings in the patient's record according to hospital protocols. Prepares patients for transportation to other departments/areas. Assists patient to cart or wheelchair. Has patient chart readily available. Transports patient when STAT or as necessary. Completes hospital forms per protocol. Assists with patient admission, discharge and transfer. Settles patients appropriately (includes positioning). Orients patient to room/routine to include Care Advisor and Virtually Integrated Care (VIC). Assists staff/patient with discharge preparations.
  • Provide appropriate Skin Care based on patient risk. Maintain skin integrity. Keep skin clean and appropriately hydrated. Cleanse the skin following episodes of incontinence. Apply non-medicated moisture barrier products to protect the skin. Suspend the heels off the bed with pillows or use heel offloading devices or boots. For incontinence, use products and devices to keep skin protected from moisture (e.g. skin barriers, urinary and fecal management devices), as indicated for the patient's condition. Limit layers of linen beneath the patient. Participates in accurate and informative nursing report and/or initiates actions that enable a timely completion of report. Assists with patient team/answers all call lights promptly, while RN gives report
  • Provides information/input for accurate report. Knocks/asks permission to enter patient rooms & closes door/draws curtain for privacy. Discontinues Foley catheters and peripheral IVs as directed by RN. Arranges patient translation device when needed. Assists patients activities of daily living according to standards of care and hospital protocol. Collects laboratory specimens according to hospital protocol, maintaining hospital standard of zero tolerance for mislabeled specimens. Reports significant observed or reported alterations in the patient's condition to the RN, physician, supervisor, or nurse manager. Reviews and implements the multi-disciplinary Plan of Care for each patient as indicated and within scope. Collaborates with RN in implementation of plan
  • Provides input to RN for inclusion in plan of care. Addresses the physiological, psychosocial and spiritual needs of each patient. Contacts RN, physicians, social workers/case management, and/or chaplain services as appropriate. Follows YRMC policy and protocol to utilize patient restraints, while maintaining the least restrictive environment as possible. Provides for patient safety/comfort at all times
  • Clearly documents all interventions. Monitors and records patient vital signs, weight, positioning, ADLs, I&Os, activity and blood glucose readings per RN instructions, hospital protocols, and standards of care. PCT reports ALL abnormal range results to RN immediately. Maintains oxygen therapy as ordered (may NOT titrate oxygen). Records readings in accurate and timely manner
  • Patient Experience Focus: Patient Advocacy: Promptly answers patient call bells and assists patients as needed. Follow the "No Pass Zone" initiative by not passing up a room with a call light on (regardless of who is assigned the room) without answering the light and assisting patient as needed. Advocating for patient comfort, needs and safety during their stay. Acting as liaison between the patient and the RN staff.

Job Requirements

Required

  • Completion of nursing assistant program, or EMT program, or completion of first block of nursing program
  • Minimum of two (2) years experience as an nursing assistant in an acute care hospital setting
  • Certified Nursing Assistant (CNA:AZ) or
  • Emergency Medical Technician (EMT:AZ)
  • Basic Life Support - CPR, (AHA BLS)
