Job Summary and Responsibilities

As our Home Health Registered Nurse (RN), you will help mature adults throughout the Sacramento Service area receive skilled treatment and education so they can reach maximum wellness and manage their care independently in their homes. You will be supported by dynamic, caring multidisciplinary clinical teams and have access to cutting-edge laptop clinical documentation systems.

Every day you will visit patients in their residences to provide hands-on skilled care and education, utilizing timely, high-tech access to patient information. You will be expected to demonstrate competency in patient care across a diverse age range, including neonate, pediatric, adult, geriatric, and thanatology patients, as directed by a multidisciplinary plan of care plan. Our Home Health nurses enjoy flexible work schedules and geographically defined caseloads.

To be successful in this role you must possess strong clinical knowledge, a desire for autonomy, the ability to effectively educate patients, and be adaptable to a supportive environment offering individualized orientation and experienced leadership.

  • Completes and submits documentation following established guidelines, as measured by a designated evaluation tool and physician orders are present and written correctly. 
  • Reviews and documents clinical progress toward goals and documents follow-up of identified problems. 
  • Demonstrates coordination of care by appropriate communication with other disciplines and documents that communication, as measured by a designated evaluation tool. 
  • Meets agency/regulatory requirements as measured by a designated evaluation tool (Adverse Outcome Reports, etc.) 
  • Documents appropriate management of visit utilization and authorization status as measured by a designated evaluation tool. 
  • Completes and submits re-certification documentation following established guidelines as measured by a designated evaluation tool.

Job Requirements

Required

  • One (1) year home health nursing (LVN or RN) experience in the past five (5) years.
  • Active Registered Nurse (RN) license in the State of California
  • Basic Life Support (BLS) certification - American Heart Association
  • Current driver's license. If out of state license, consistent with CA DMV regulations, must obtain a CA driver's license within the first thirty (30) days of employment.
  • Current possession of automobile insurance and access to an operating motor vehicle.

 

Preferred

  • One (1) year in an acute care setting or ICU nursing experience in the past five (5) years.
  • BSN degree.
  • Public Health certificate
