Job Summary and Responsibilities

As our Licensed Vocational Nurse (LVN), you will provide vital direct patient care, significantly contributing to patient well-being and recovery as an integral part of our compassionate healthcare team.

 

Every day, you will assess patient conditions, administer medications, perform prescribed treatments, and meticulously document care. You will collaborate with RNs and physicians to implement individualized care plans, ensuring a seamless and high-quality patient experience.

 

To be successful in this role, you will demonstrate exceptional clinical skills, a strong commitment to patient safety, and excellent communication. Your compassionate demeanor, attention to detail, and ability to prioritize in a fast-paced environment are essential.

  • Demonstrates competency in the care of patients during field observation by the clinical supervisor.
  • Completes and submits documetation following established guidelines, as measured by defined evaluation processes (ie. chrt audits/QM reviews, etc.
  • Physician orders are written according to policy and review/co-signed by RN.
  • Documents follow-up of identified problems as clinically supervised by RN.
  • Reviews and documents clinical progress toward goals.

Job Requirements

Required

  • One (1) year of Home Health LVN experience within the last five (5) years.
  • Recent experience as an LVN in a Home Health or Hospice Agency - preferred.

Licensure:

  • Currently licensed as a LVN in California
  • Current California driver's license, possession of automobile insurance, & access to an operating motor vehicle.
  • Current CPR/BLS card approved through the American Heart Association.
