Founded in 1910, Dignity Health - Mercy Hospital is a 229-bed, acute care, nonprofit hospital located in Bakersfield, California. They are both Joint Commission-certified Primary Stroke Centers. Job Summary and Responsibilities

As our Community Health Worker, you will be a vital link between individuals and essential community resources, actively working to reduce health disparities and improve access to support services.

Every day, you will proactively engage with clients, assessing their needs and identifying pathways to care and support. You'll connect individuals to relevant health, social, educational, and employment resources, providing guidance and practical assistance to overcome logistical barriers.

To be successful in this role, you will possess a profound understanding of diverse community needs, exceptional problem-solving abilities, empathy, and resilience. Your dedication to empowering clients and removing barriers to care will be paramount.

  • Building community relationships and increasing program visibility within the community.
  • Providing education on various health topics
  • Enrolling participants in classes and workshops
  • Completing and submitting all required documents and reports to Supervisor
  • Clerical duties, such as data entry, making phone calls to participants and preparing workshop packets.

Job Requirements

Minimum Qualifications:

  • Bilingual (Spanish/English - verbal and written for both) for effective communication with patients family members and customers of all social and economic levels from a diverse cultural and ethnic population.
  • Current drivers license and car insurance
  • Basic Life Support - CPR (BLS-CPR)
  • Must be a motivated well-organized learn player.
  • Must be culturally sensitive and have excellent interpersonal communication and customer service skills.
  • Must be able to work independently stay focused on results and task orientation.
  • Prior knowledge in public health promotion perinatal services and marketing helpful.
  • Ability to utilize office equipment (i.e. photocopy machine facsimile machines and telephones.

Preferred Qualifications:

  • High School Graduate General Studies and Experience in customer service and community outreach, upon hire or
  • High School GED General Studies and, upon hire and
  • Associates Other or higher in the area of social services
  • Experience in customer service and community outreach preferred.

Please be advised that this location requires quarterly American Heart Association (AHA) Resuscitation Quality Improvement (RQI) competency certification post-hire. This ongoing competency is required through resources internal to the facility; outside RQI competency resources are not acceptable for CommonSpirit Health compliance requirements.
