Job Summary and Responsibilities

As our Social Worker, you will help patients and families navigate complex psycho-social stressors so they can achieve improved health outcomes and receive the necessary community and clinical resources.

 

Every day you will perform social work assessments, crisis interventions, and brief counseling for high-risk patients. You will be expected to collaborate with interdisciplinary healthcare teams to develop comprehensive care plans, advocate for patient needs, and facilitate post-acute placements or community resource referrals.

 

To be successful in this role, you must possess strong critical thinking skills and professional expertise in addressing social determinants of health, trauma-informed care, and end-of-life planning, while adhering to the NASW Code of Ethics.

Job Requirements

Required

  • Master's degree in Social Work
  • 2 years of relevant experience
  • Licensed Social Worker Associate Advanced:WA (LSWAA:WA)

    or

    Provisional Licensed Master Social Worker: WA (PLMSW:WA)

    or

    Licensed Social Work Advanced: WA (SWORKADVLI:WA)

    or

    Master Social Worker: WA (MSW:WA)

    or

    Licensed Advanced Social Worker: WA (LASW:WA)

    or

    Licensed Independent Clinical Social Worker: WA (LICSW:WA)
