Job Summary and Responsibilities

As our Home Care Supervisor, you will help our home care professionals deliver high-quality patient care so they can ensure the health and well-being of our diverse clientele throughout the Sacramento Service area.

Every day you will supervise and motivate an interdisciplinary team, acting as a clinical resource, monitoring performance, and ensuring compliance with all regulatory requirements. You will be expected to collaborate with a wide range of stakeholders to optimize patient care outcomes and achieve department-specific metrics.

To be successful in this role you must possess strong clinical decision-making skills, excellent communication and interpersonal abilities, and a comprehensive understanding of home health regulations and quality improvement strategies.

  • Collaborates with other area supervisors as required to support the hospital's role as a community health service provider. 
  • Assists in implementing a unit Quality Management Program consistent with hospital wide Performance Improvement Plan. Operates the units within the financial and productivity guidelines, setting goals and developing policies that support this objective. 
  • Provides for quality services by ensuring that the staff are well trained in the services to be provided. 
  • Addresses, escalates or resolves unit issues, including problem resolution related to customer complaints, concerns and suggestions.

 

Job Requirements

Required

  • Minimum of two (2) years of management/leadership experience (within the last 5 years) in a home health agency, primary care clinic or health facility, sufficient background knowledge and expertise in clinical decision making for the patient population assigned.
  • Active Registered Nurse (RN) license in the State of California
  • Basic Life Support (BLS) certification - American Heart Association
  • Associates Degree in Nursing

 

Preferred

  • Home Health Supervisory experience.
