As our Program Manager, Community Health, you will lead and drive strategic initiatives designed to improve community health outcomes and foster wellness. You will be the key liaison between the organization and the community, responsible for implementing health programs that address critical needs such as diabetes, obesity, and preventative care.
Every day you will coordinate, implement, and evaluate community health education resources and support services to meet the identified needs of the community. You will engage directly with individuals, families, and community groups, providing one-on-one coaching and health education while collaborating with clinical providers to ensure that all messaging and care are culturally and linguistically appropriate.
To be successful in this role, you will possess a passion for health equity and a strong background in community outreach or public health program management. A Bachelor’s degree in a relevant field is preferred, though relevant experience is highly valued.
