CHI Health St. Francis was established in Grand Island, Neb. in 1887. Today, our hospital is a regional referral center, with more than 100 physicians and more than 1,100 team members working together to build a healthier community. As our Social Worker, you'll play a vital role in helping patients and families navigate complex healthcare needs with compassion and expertise. By addressing social, emotional, financial, and discharge planning needs, you'll help remove barriers to care and support positive patient outcomes throughout the care journey.
Every day you will collaborate with patients, families, physicians, nurses, and the interdisciplinary care team to develop individualized plans of care and coordinate safe, timely transitions. You'll serve as a trusted advocate, connecting patients with community resources and support services that promote continued health and well-being beyond the hospital.
To be successful in this role, you will bring strong clinical assessment, communication, and problem-solving skills along with a passion for patient advocacy. You'll thrive in a collaborative environment where your ability to build relationships, navigate community resources, and support patients through challenging situations makes a meaningful difference every day.
