Job Summary: As a member of a multidisciplinary team, in a leadership role, is responsible for coordination of patient care along the full continuum and/or for a target population.  Through intervention, promotes the appropriate utilization of resources, identifies and addresses care gaps, promotes health behavior changes for patients to better manage their illnesses and recovery (if not chronic), and analyses readmissions to the acute setting post hospitalization.  As a result, optimal clinical, financial, functional and health status improvements in the target population will be accomplished.  Incumbents will use professional judgment, independent analysis and critical-thinking skills to apply clinical guidelines, policies, and regulatory requirements, etc. to determine the appropriate level of care.  Monitor’s patient’s clinical status and needs in order to ensure appropriate intensity of services and anticipate and prepare for transitions of care.  Interventions will be targeted towards effective management of each episode of care and the seamless transition to the next level of care.  The goal will be towards promoting patient engagement and participation in their care at their highest attainable level.  The desired result is management of the patient in the least restrictive environment possible.    Incumbents are also accountable for demonstrating a strong commitment to promoting excellence every day by maintaining and demonstrating competent professional practice and engaging in opportunities for professional growth. Manages daily staffing and work assignments utilizing approved staffing metrics. 7. Provides work instruction and assists employees with difficult and/or unusual assignments, encouraging innovation. Facilitates problem resolution and mediates conflicts encountered during daily operations, determines appropriate solutions, and promotes teamwork. 8. GENERAL SUMMARY:    The RN Clinical Coordinator for Cancer Survivorship provides innovative and collaborative coordination of patient care in the transition from treatment to survivorship stage/follow-up. This position will manage the care of patients with a history of cancer that are making the transition to life-after-treatment in conjunction with the patient’s multi-disciplinary care team, according to agreed-upon clinical guidelines, and with the objective of coordinating high quality patient care across all delivery sites.   ESSENTIAL FUNCTIONS: - Coordinate patient care following active cancer treatment, in collaboration with physicians and the broader care delivery team and across the continuum of care (oncology clinic offices, primary care clinic offices, palliative care, hospice, inpatient, ancillary services, and community resources). - Educate patient on methods to continue surveillance for cancer recurrence or new primaries. - Educate patient on prevention and detection of new cancers. - Serve as the primary point of contact between Cancer Resource Center resources and patients. - Guide patients on interventions for long-term and late effects from cancer and its therapies. - Coordinate between specialists and primary care providers to ensure that all of the survivor’s needs are met. - Interface with other healthcare teams for appropriate referrals/services such as the social worker, pastoral care, dietician, dental hygienist, financial counselor, cancer rehabilitation, clinical research nurses, survivorship services, palliative care, hospice, smoking cessation, etc. - Build partnerships with local agencies and groups for community services referrals (e.g., referrals to other services and/or cancer survivor support groups). - Coordinate completion of treatment summaries and survivorship care plans for designated disease sites. Effectively utilize information system tools for creation and delivery of these summaries / plans to patients and other members of the care team. - Document all patient coordination activities, education, and other interactions in designated inpatient and outpatient electronic health record systems to assure a timely, complete patient record and effective communication with other members of the care delivery team. Assist with tracking, documentation and outcome reporting for survivorship services. - Collaborate with other members of the care delivery team to assist in developing programming for individuals and groups on topics of prevention, detection, surveillance, and strategies for dealing with the after effects of the disease. - Plan and coordinate community screening activities and other community outreach / educational activities. Conduct health promotion and awareness programs and presentations in community as appropriate. Serve as a resource for and attend community health fairs and screenings. - Serve as a liaison with community advocacy groups to promote Mercy Cancer Center (e.g., American Cancer Society, American Lung Association, Iowa Cancer Consortium). Represent Mercy Cancer Center in committees and activities of these groups.  - Assist with quality improvement efforts for designated disease sites (complete quality studies, set and monitor quality improvement goals, modify patient care protocols based on QI studies). - Provide staff education (e.g., Ovarian Conference, symposiums). - Serve as a role model and mentor in promoting survivorship concepts to Mercy Cancer Center, Mercy Medical Center, Mercy Health Network, general community, long term care facilities, and various clinics. #missioncritical
